Your child has been selected to participate in an internship at Ty Stephano, located at 7517 Melrose Ave, Los Angeles, California. This internship is designed to teach essential sewing techniques and introduce students to the world of high-fashion garment creation. The internship will include hands-on training in a professional atelier setting, supervised by our expert staff. We kindly request your permission for your child to participate in this educational opportunity. Parental/Guardian Consent: By signing this form, you are giving your consent for your child to participate in the Ty Stephano Sewing Class Internship. You acknowledge that your child will be operating in a professional sewing atelier, and while all safety measures are in place, you understand the risks associated with the use of sewing equipment. Health and Safety: We prioritize the safety and well-being of all our interns. Safety measures, including proper use of sewing equipment and materials, will be strictly enforced. First-aid will be readily available on-site. Release of Liability: I, the undersigned, hereby release Ty Stephano, its staff, and all affiliated personnel from any claims or liabilities arising out of or connected to my child’s participation in the internship. I understand the inherent risks of working in a sewing atelier and agree to assume all responsibility for any injury or incident. Monthly Payment: A monthly payment of $600 is required to cover the cost of materials and teaching lessons. This payment ensures that your child has access to the necessary resources and guidance throughout the internship program.
